Basic Life Insurance - Various multiples of earnings
- Flat amounts (minimum $10,000)
- Reduction: 50% at age 65
- Termination at age 65, 70 or earlier retirement
-Benefit Maximum: $ 1,000,000

Accidental Death and Dismemberment Insurance - Same as basic life amount
- "Loss Of" and "Loss of Use" benefits" are included
- 2 x life amount is available

Dependent Life Insurance - Spouse: From $2,000 to $25,000
- Child: 50% of spousal coverage
- Effective coverage: From birth or after 24 hours

Optional Life Insurance - Coverage in increments of $10,000 - Available for employee and spouse

Optional AD&D - Same as optional life insurances
DISABILITY INSURANCE
Short Term Disability Percentage of salary Waiting & Benefit Periods
- 50, 55, 60% of weekly salary - Accident: 0, 3, 7, 14 or 30 days
- 66.67% of 1st $500 of weekly salary, - Sickness: 3, 7, 14 or 30 days
50% of next $500 & 40% of balance - First day of hospitalization available

Long Term Disability Definition of Total Disability
